This role will assist each participant to achieve their goals by enabling increased access to appropriate services and tailored recovery and support planning.Other key duties include:Deliver services providing increased opportunities for recovery for people aged 16 years and over whose lives are severely affected by mental illness, by helping them to overcome social isolation and increase their connections to the community.Support of transitioning clients effectively to the NDIS, including mental health assessment plans, access requests and support packagesUndertake an active and client outcome focused case management approach, based upon individual recovery plans, and actively seek & source supports from relevant agencies to meet consumer needsProvide direct and personalised assistance through outreach services About YouThe successful applicant will have an understanding of mental health and disability services as well as a knowledge of the National Disability Insurance Scheme (NDIS). You will also hold a minimum Certificate IV Mental Health or Community Services or equivalent and be able to provide a high level of service to both internal and external customers.You will also come with:The ability to quickly build rapport & conduct objective assessment of the client’s needs, mental health & abilitiesDemonstrated ability to manage high level workloads and meet project deadlines and outcomesExperience maintaining compliance with contractual requirementsCompetency in computer applications including Microsoft OfficeA current and valid driver’s licence is essential, as is a current Blue Card or the willingness to gain one.
