The program draws upon Strength and Recovery Model principles to develop comprehensive individualised care plans that assist individuals to improve their health and wellbeing, community connection and access to specialist services. The RoleWe are now looking to hire a full time Peer Support Worker (contract till June 2019) who will utilise their lived experience to support individuals in recovery based principles. This will include providing one on one support to identify barriers impacting upon mental health and quality of life. This role will support participants to achieve greater independence, socialisation, education and other personal goals.The Peer Support Worker will actively assist individual with severe and persistent mental illness to proactively access supports through the National Disability Insurance Scheme.Other key duties include:Deliver services providing increased opportunities for recovery for people aged 16 years and over whose lives are severely affected by mental illness, by helping them to overcome social isolation and increase their connections to the community.Actively support PHaMs participants to apply for access into the National Disability Insurance Scheme (NDIS) in line with the full rollout of the scheme.Undertake individual assessments, and provide support for people with mental health concerns that is strengths based, consumer directed and recovery focused to help them achieve personal goals and manage everyday tasks.Provide direct and personalised assistance through outreach services About YouThe successful applicant will utilise their lived mental health experience with participants as well as within team discussions where appropriate, to inform and strengthen the quality of services provided to participants. As well as an understanding of mental health, a knowledge of the National Disability Insurance Scheme (NDIS) is preferred but not essential. You will also hold a relevant qualification such as minimum Certificate IV Mental Health or Community Services or otherwise be willing to obtain this certification upon appointment.You will also come with:The ability to recognise and support people experiencing barriers due to their Mental HealthHigh levels of service to both internal and external customersDemonstrated openness and responsiveness to changeExperience maintaining compliance with contractual requirementsCompetency in computer applications including Microsoft OfficeA current and valid driver’s licence is essential, as is a current Blue Card or the willingness to gain one.
